READ CAREFULLY 

 

By making a payment to enter TheVictorRojas V-Class you understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury and, in rare circumstances, death.

 

You agree to release and hold harmless TheVictorRojas V-Class and Victor Rojas, including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future.

 

You will not hold TheVictorRojas V-Class or Victor Rojas liable for any personal injury or any personal property damage, which may occur. Furthermore, you understand that you should be aware of your, and/or your child’s physical limitations and agree not to exceed them.

 

If you are reading this waiver for your child, you certify that you are the parent or legal guardian and have the right to waive these rights.

 

Further, you authorize and agree that TheVictorRojas V-Class and Victor Rojas may take and use photographs/videos of you and/or your child during the class for purposes of record keeping, advertising, and marketing. You understand that you do not have any rights to these photographs/videos and will not be compensated for the same.

 

PAYMENT IS PER STUDENT EVEN IF MORE THAN ONE PERSON IS IN THE ROOM.

 

If for any reason you do not agree with these terms and conditions outlined in this liability waiver it is your responsibility to contact thevictorrojas@yahoo.com PRIOR to the date of class to request a full refund.

 

NO REFUNDS WILL BE GIVEN THE DAY OF CLASS. NO EXCEPTIONS.
